REGISTRATION
Who should attend?
All practicing and academic Physiatrists, Post graduate students in PMR, Physicians, Orthopaedic Surgeons, Neurologists, Paediatricians, Rheumatologists and General Practitioners with special interest in PMR will find this Conference extremely useful.
Please fill in the Registration form and send it to the Organizing Secretary along with registration fees, accommodation charges (one day's charge in advance) and other charges, if any. Only demand drafts in favour of "IAPMRCON 2004" payable at Calicut, Kerala will be accepted.Personal Cheques will not be accepted. Alternatively, you can remit the charges directly to our Account in any ICICI Bank in India .
Kindly write the details of payment on the Registration form.You can send it as e mail attachment to the Organizing Secretary if payment has been made in ICICI Bank.Kindly send the form by post if you prefer to pay by Demand Draft.

Registered delegates are allowed to participate in all scientific sessions, cultural programs, inaugural and valedictory functions. Registration charges include breakfast, lunch, evening tea, dinner and banquet. To and fro transportation between Conference venue and place of stay, Conference kits and Gifts also are included in this package.Spot registrants cannot be assured of Conference kits and Gifts.
Post graduate students must send along with the Registration form a valid Certificate from the Head of the Institution where they are undergoing training indicating their post graduate training status.
Details of Spouse and Associate delegates are to be included in the Registration form. Please don't forget to add their registration charges as well.
Sponsored delegates are requested to mail the Registration form duly filled in to the Organizing Secretary. Please see the Sponsorships page.
 
Refund and Cancellation
Please don't cancel your registration !!! Well...if there is no other alternative...
Please inform the Organizing Secretary in writing about cancellation on or before 15 . 01. 2004. The cancellation fee is Rs. 400 /-(Rupees four hundred only). For overseas delegates it will be 50 US$. No refund will be made if the request is received after 15. 01. 2004 or if the Delegate fails to attend the Conference. Accommodation and other charges if already paid to the Hotel / Place of stay will not be refunded. Refund of the Registration fee will be made only after the Conference is over.
